In order to save you time on your visit and help us to stay on time for your appointment, please take a few moments now to gather the items you will need and register or update your records using the secure patient portal for the digital medical record system.

When you made your appointment, you were given an office code that you will need to complete the digital registration/ update process. Returning patients can login with the username that you provided during the initial registration process.

If you do not have access to a computer or if you need assistance, we will gladly help you through the process at our lobby computer. This does take some time, so please arrive at least 30 minutes prior to your appointment. If you have already completed the digital chart, please arrive 15 minutes prior to your appointment.

There will be additional forms that we will ask you to complete when you arrive for your appointment. Please bring the following:

Current insurance card
List of current medications (including vitamins and
supplements)
Physician referral if needed
Copies of any pertinent diagnostic tests or medical records.
